16 Essential Questions: Can you tell me about the surgery?

At True & Dorin, we believe in providing the best possible treatment for each patient.

We ask patients to come into the clinic having eaten a light breakfast and taking a first dose of an antibiotic on the day of the surgical procedure. Upon arrival , we’ll complete  formal intake paperwork and take some baseline photographs for our medical record. We’ll then review medical history and take  vital signs before beginning the procedure.

After ensuring acceptable status , a patient is administered  a  mild tranquilizer by mouth, typically Valium,  as it produces some mild relaxation. The tranquilizer is worn off by end of the day. After that, we prepare the donor area by shaving a narrow band of hair in the donor zone. In this process, we use CompuMed, computerized local anesthesia system to anesthetize the donor area. Once this process gets completed, the doctor removes and sutures the donor tissue and an experienced team of nurses and other medical staff start preparing for the individual grafts with dissecting steromicroscopes. Generally, the process of graft preparation takes around one to three hours to complete, depending upon the size of the case.

While the grafts are being prepared, the doctor and patient have a design session during which the distibution of grafts is mapped out and if pertinent the hairline is designed.

The part of the scalp to be transplanted is then anesthetized with the Compumed.

Once everything is set, the doctor starts making all of the tiny little micro-incisions. While working on the decided plan, we use custom  micro blades that can be  from 0.7 millimeters to 1.1 millimeters wide. We measure each patients follicles to determine prcisely the depth and width of microincision required to be minimally invasive and allow the highest density possible (can be as much as 60 grafts per square centimenter).

We also do some critical work that includes inspecting the patient’s grafts through the microscope and the follicles to determine the exact size that we want to use for that person. We also determine the depth so that we can get these follicles as close as possible within the scope of the treatment and also have them placed at the right level.  This approach will ensure the best healing of the skin.  There are no bumps, there are no pits.

To ensure the desired results, we do grafting by hand without using any kind of mechanical help. The process of hand-grafting is very meticulous and thus, it requires highly-skilled efforts. Therefore, only highly skilled and trained nurses’ and technicians’ are allowed to perform this part of the procedure.

Usually, two or three people work together to place the grafts. To comfort the patient undergoing the surgery, the anesthesia is maintained throughout the procedure. However, we do provide periodic stretch, rest room and lunch breaks as time goes along. At the end of the procedure, the patient is shown a DVD featuring the entire post-operative care that needed to be taken care of.

At the end of the procedure, the surgeon checks in the success of the procedure, gives instructions that needed to be followed and then makes appointments for the post-operative visits at that point.
